BRIDGE PROGRAM
Overview
The
Bridge Program is designed to provide a smooth and successful transition
into the Supported Living Program. This program is open to recent
graduates, as well as clients entering the Supported Living Program
from other programs.

Program Client Profile
New
Clients The program is ideal for newly enrolled Chapel
Haven clients who have previously lived independently or have had
some independent living training. For these individuals, the program
is an introduction to the Chapel Haven and Greater New Haven communities
and offers reinforcement of life skills in a structured setting.
Recent Graduates
The program is also designed for individuals who
recently graduated from the REACH Program. Each year, Chapel Haven
staff may identify a small number of second-year residential clients
who will benefit from a year long transition into independent
living.
Program
Goals
Bridge
participants live for one year in Chapel Haven owned properties
on campus. A Coordinating Teacher oversees students and the Life Skills Instructors provide one-to-one and small group services
based on individual needs. Assistance includes help with housekeeping,
budgeting and banking, menu planning, health and wellness, interpersonal
relationships and community activities. The program emphasizes
community integration, creating and adhering to schedules, and social
and relationship issues while
incorporating life skills training. The goal of the program
is to reduce the amount of support needed by the client once he
or she enters the Supported Living Program.

Tuition for the Bridge Program includes the following:
